‘First up is one of the more complex and intriguing cases referred to me in the last 2 years.’


This Vertucci type V LR4 case had been previously initiated by the referring dentist (RD) and displayed minimal apical change on the pre-op IOPA.
Once I had carried out a consultation and taken my own IOPAs, including a mesial angle shift, I felt the root treatment had an excellent chance of success for this tooth. Usual local anaesthetic rubber/liquid dam protocols were used. The tooth was reaccessed using a combination of ultrasonic access tips and LG burs to improve straight-line access to the deep-lying apical split. Preparation was completed using Dentsply’s Reciproc Blue R25 file using lengths calculated from the Morita ZX apex locator (zero readings -0.5mm). Copious irrigation was performed with 5.25% NaOCl and 17% liquid EDTA.
All irrigants were activated using passive ultrasonic irrigation (PUI) with Eighteeth’s Ultra X. It was then filled using a combination of warm vertical compaction (WVC) and AH+ resin-based sealer. 3M’s Filtek’s bulk one resin was used as a nayyar core. This lovely lady was then returned to her RD to provide cuspal coverage.
